Overview
Home health care plays an important role in a patient’s recovery. It allows patients to receive needed health related services in the comfort of their own home, surrounded by their family, friends and a supportive health care team. Our multi-disciplinary team will focus on helping patients and their families become independent in their care and remain independent in their home environment.
Home health care may be used to learn:

  • About disease processes and medications
  • How to manage ongoing health issues

  • How to do wound care, promote healing and prevent skin breakdown

At Carondelet Home Care Services, our team of professionals works under the direction of the patient’s physician to provide a wide range of services to meet the needs of each patient.
